Taking Out the Torah Scroll
אלו ימים שאין אומרים בהם "אל ארך אפים": ראש חודש, חנוכה ופורים קטן וגדול ב' ימים, ערב פסח, תשעה באב ולא בבית האבל:
These are the days when 'God, slow to anger' is not said: Rosh Chodesh, Hanukkah, and Purim Katan and Gadol for both days, Erev Pesach, Tisha B’Av, and not in the house of a mourner.
אֵל אֶרֶךְ אַפַּיִם וְרַב חֶסֶד וֶאֱמֶת. אַל בְּאַפְּךָ תוכִיחֵנוּ. חוּסָה ה' עַל יִשרָאֵל עַמֶּךָ וְהושִׁיעֵנוּ מִכָּל רָע. חָטָאנוּ לְךָ אָדון. סְלַח נָא כְּרוב רַחֲמֶיךָ אֵל:
God, slow to anger and abounding in kindness and truth, do not rebuke us in Your anger; spare, Lord, Israel Your people, and save us from all evil; we have sinned against You, Master; forgive us, please, according to the abundance of Your mercies, God:
וַיְהִי בִּנְסעַ הָאָרן וַיּאמֶר משֶׁה קוּמָה ה' וְיָפֻצוּ איְבֶיךָ וְיָנֻסוּ מְשנְאֶיךָ מִפָּנֶיךָ: כִּי מִצִּיּון תֵּצֵא תורָה וּדְבַר ה' מִירוּשָׁלָיִם:
בָּרוּךְ שֶׁנָּתַן תּורָה לְעַמּו יִשרָאֵל בִּקְדֻשָּׁתו:
And it was when the ark set out, Moses said: Rise up, Lord, and let Your enemies be scattered, and let those who hate You flee from before You; for from Zion shall go forth Torah, and the word of the Lord from Jerusalem:
Blessed is He who gave the Torah to His people Israel in His holiness:
בְּרִיךְ שְׁמֵהּ דְּמָרֵא עָלְמָא. בְּרִיךְ כִּתְרָךְ וְאַתְרָךְ. יְהֵא רְעוּתָךְ עִם עַמָּךְ יִשרָאֵל לְעָלַם. וּפֻרְקַן יְמִינָךְ אַחֲזֵי לְעַמָּךְ בְּבֵית מַקְדְּשָׁךְ. וּלְאַמְטויֵי לָנָא מִטּוּב נְהורָךְ וּלְקַבֵּל צְלותָנָא בְּרַחֲמִין. יְהֵא רַעֲוָא קֳדָמָךְ דְּתורִיךְ לָן חַיִּין בְּטִיבוּ. וְלֶהֱוֵי אֲנָא פְקִידָא בְּגו צַדִּיקַיָּא. לְמִרְחַם עָלַי וּלְמִנְטַר יָתִי וְיַת כָּל דִּי לִי וְדִי לְעַמָּךְ יִשרָאֵל. אַנְתְּ הוּא זָן לְכלָּא וּמְפַרְנֵס לְכלָּא. אַנְתְּ הוּא שַׁלִּיט עַל כּלָּא. אַנְתְּ הוּא דְשַׁלִּיט עַל מַלְכַיָּא. וּמַלְכוּתָא דִּילָךְ הִיא. אֲנָא עַבְדָּא דְקֻדְשָׁא בְּרִיךְ הוּא דְּסָגִידְנָא קַמֵּהּ וּמִקַּמֵּי דִּיקַר אורַיְתֵהּ בְּכָל עִדָּן וְעִדָּן. לָא עַל אֱנָשׁ רָחִיצְנָא וְלָא עַל בַּר אֱלָהִין סָמִיכְנָא. אֶלָּא בֶּאֱלָהָא דִשְׁמַיָּא. דְּהוּא אֱלָהָא קְשׁוט וְאורַיְתֵהּ קְשׁוט וּנְבִיאוהִי קְשׁוט. וּמַסְגֵּא לְמֶעְבַּד טַבְוָן וּקְשׁוט. בֵּהּ אֲנָא רָחִיץ. וְלִשְׁמֵהּ יַקִּירָא קַדִּישָׁא אֲנָא אֵמַר תֻּשְׁבְּחָן. יְהֵא רַעֲוָא קֳדָמָךְ דְּתִפְתַּח לִבָּאִי בְּאורַיְתָא. וְתַשְׁלִים מִשְׁאֲלִין דְּלִבָּאי וְלִבָּא דְכָל עַמָּךְ יִשרָאֵל. לְטַב וּלְחַיִּין וְלִשְׁלָם:
Blessed be the name of the Master of the universe; blessed be Your crown and Your place; may Your favor be with Your people Israel forever; and may the redemption of Your right hand be shown to Your people in Your sanctuary; and bring us a portion of the goodness of Your light, and accept our prayers with mercy; may it be Your will that You sustain us with life in goodness; and may I be counted among the righteous, to find mercy upon me and to guard me and all that is mine and that belongs to Your people Israel; You are the One who nourishes all and sustains all; You are the One who rules over all; You are the One who rules over kings, and the kingdom is Yours; I am a servant of the Holy One, blessed be He, before whom I bow and before the glory of His Torah at all times; not in man do I trust, nor on any divine being do I rely, but in the God of heaven, who is the God of truth, and His Torah is truth, and His prophets are truth, and He abounds in doing goodness and truth; in Him I trust, and to His holy and glorious name I offer praises; may it be Your will that You open my heart to the Torah, and fulfill the requests of my heart and the heart of all Your people Israel for good, for life, and for peace:

אַב הָרַחֲמִים. הוּא יְרַחֵם עַם עֲמוּסִים וְיִזְכּור בְּרִית אֵיתָנִים. וְיַצִּיל נַפְשׁותֵינוּ מִן הַשָּׁעות הָרָעות. וְיִגְעַר בְּיֵצֶר הָרָע מִן הַנְּשוּאִים. וְיָחון אותָנוּ לִפְלֵיטַת עולָמִים. וִימַלֵּא מִשְׁאֲלותֵינוּ בְּמִדָּה טובָה יְשׁוּעָה וְרַחֲמִים:
Father of mercies, may He have mercy on a burdened people and remember the covenant of the mighty ones; and may He save our souls from evil times; and may He rebuke the evil inclination from those who are married; and may He favor us with eternal deliverance; and may He fulfill our requests with a good measure, salvation, and mercy:
וְתִגָּלֶה וְתֵרָאֶה מַלְכוּתו עָלֵינוּ בִּזְמַן קָרוב. וְיָחון פְּלֵטָתֵנוּ וּפְלֵטַת עַמּו בֵּית יִשרָאֵל לְחֵן וּלְחֶסֶד וּלְרַחֲמִים וּלְרָצון וְנאמַר אָמֵן. הַכּל הָבוּ גדֶל לֵאלהֵינוּ וּתְנוּ כָבוד לַתּורָה. כּהֵן קְרָב יַעֲמד [פלוני בן פלוני] הַכּהֵן. בָּרוּךְ שֶׁנָּתַן תּורָה לְעַמּו יִשרָאֵל בִּקְדֻשָּׁתו:
And may His kingdom be revealed and seen over us soon; and may He favor our remnant and the remnant of His people, the house of Israel, with grace, kindness, mercy, and favor, and let us say, Amen; let all ascribe greatness to our God and give honor to the Torah; let the Kohen approach, let [so-and-so son of so-and-so] the Kohen stand; Blessed is He who gave the Torah to His people Israel in His holiness:

If a young man who has become Bar Mitzvah is called to the Torah, then after he recites the final blessing, his father says this:
בָּרוּךְ שֶׁפְּטָרַנִי מֵעָנְשׁו שֶׁלָּזֶה:
Blessed is He who has relieved me of the punishment of this one:

In the world that He created according to His will, and may He establish His kingdom in your lifetime and in your days, and in the lifetime of the entire house of Israel, speedily and soon, and say, Amen; [Congregation: Amen]

Beyond all blessings and hymns, praises and consolations that are uttered in the world; and say, Amen; [Congregation: Amen]
וְזאת הַתּורָה אֲשֶׁר שם משֶׁה לִפְנֵי בְּנֵי יִשרָאֵל. עַל פִּי ה'בְּיַד משֶׁה:
And this is the Torah that Moses placed before the children of Israel, by the mouth of the Lord, through the hand of Moses:

Returning the Torah Scroll
כשמוליכין הס"ת לארון אומר החזן:
When the Torah scroll is carried to the ark, the cantor says:
יְהַלְלוּ אֶת שֵׁם ה'. כִּי נִשגָּב שְׁמו לְבַדּו:
Let them praise the name of the Lord, for His name alone is exalted:
והקהל אומרים:
And the congregation says:
הודו עַל אֶרֶץ וְשָׁמָיִם: וַיָּרֶם קֶרֶן לְעַמּו. תְּהִלָּה לְכָל חֲסִידָיו. לִבְנֵי יִשרָאֵל עַם קְרובו. הַלְלוּיָהּ
His splendor is above earth and heaven; and He has raised a horn for His people, a praise for all His pious ones, for the children of Israel, a people near to Him; Hallelujah:

וּבְנֻחה יאמַר שׁוּבָה ה' רִבְבות אַלְפֵי יִשרָאֵל: קוּמָה ה' לִמְנוּחָתֶךָ. אַתָּה וַאֲרון עֻזֶּךָ: כּהֲנֶיךָ יִלְבְּשׁוּ צֶדֶק. וַחֲסִידֶיךָ יְרַנֵּנוּ: בַּעֲבוּר דָּוִד עַבְדֶּךָ אַל תָּשֵׁב פְּנֵי מְשִׁיחֶךָ: כִּי לֶקַח טוב נָתַתִּי לָכֶם. תּורָתִי אַל תַּעֲזבוּ: עֵץ חַיִּים הִיא לַמַּחֲזִיקִים בָּהּ. וְתמְכֶיהָ מְאֻשָּׁר: דְּרָכֶיהָ דַרְכֵי נעַם וְכָל נְתִיבותֶיהָ שָׁלום: הֲשִׁיבֵנוּ ה' אֵלֶיךָ וְנָשׁוּבָה. חַדֵּשׁ יָמֵינוּ כְּקֶדֶם:
And when it rests, he says: Return, Lord, to the myriads of the thousands of Israel; rise up, Lord, to Your resting place, You and the ark of Your strength; let Your priests be clothed with righteousness, and let Your pious ones sing for joy; for the sake of David Your servant, do not turn away the face of Your anointed; for I have given you a good teaching; do not forsake My Torah; it is a tree of life to those who grasp it, and those who uphold it are happy; its ways are ways of pleasantness, and all its paths are peace; return us, Lord, to You, and we will return; renew our days as of old.
